Health Equity Initiative (HEI)
West Modesto Community Collaborative is committed to advancing health equity by addressing the social, environmental, and systemic factors that impact the well-being of families in West and South Modesto and surrounding communities.
As a partner and active member of the Stanislaus Health Coalition, WMCC participates in all three Action Work Groups (AWGs) that emerged from the County’s Community Health Assessment (CHA) and now inform the Community Health Improvement Plan (CHIP). We are actively engaged in the:
- Behavioral Health AWG
- Chronic Disease AWG
- Healthcare Access AWG
Through these collaborative efforts, we help identify community priorities, elevate resident voice, and support coordinated strategies that improve long-term health outcomes across Stanislaus County.

Health Coverage Access
Connecting our community to health coverage and resources
West Modesto Community Collaborative (WMCC) is partnering with Kaiser Permanente to help eligible individuals and families access health coverage through the Community Health Coverage Program (CHCP).
CHCP is designed to help individuals and families who may not qualify for Medi-Cal or have access to other affordable health coverage options.
Who May Qualify?
CHCP may be an option for individuals and families who:
- Do not have access to affordable health coverage.
- Do not qualify for Medi-Cal or other qualifying health coverage programs.
- Meet Kaiser Permanente's CHCP income, residency, and other eligibility requirements.
How Can WMCC Help?
WMCC provides community-based assistance to help residents learn about and apply for CHCP, including:
- Program Information: Learn about CHCP and eligibility requirements.
- Application Assistance: Receive guidance with application steps and required documentation.
- Enrollment Support: Get assistance with follow-up and next steps in the enrollment process.
